Employment Forms

Welcome to the staff of Plantation Farm Camp!  This page is for people who have already been hired, and contains the forms you need to complete along with instructions. If you are looking to apply for a job, please do not complete these forms--yet. Rather, click here to read about the application process.

Together, we are about to embark on a wonderful journey together.  We will create a cohesive, supportive, fun community that will have a powerfully positive impact on our campers.

Forms

There are quite a number of forms to complete. Some are necessary for you to do your job, others are required by law.  We have also included some information that will make your summer at camp more comfortable and enjoyable.

Below are the forms together with instructions. There is a section for Program Staff and a separate section for Support Staff. Some forms you need to complete; others are simply for your information and you do not need to fill anything out.

After you have completed the necessary forms, please sign the acknowledgment at the bottom of your employment agreement letter and return that letter to Camp. We need the forms BEFORE YOU ARRIVE AT CAMP  and some are required right away. If you have any questions or have problems opening any of the links, please call us. For All Staff

1. Employee Manual: The Employee Manual covers basic camp policies and procedures and contains critical information for which you will be responsible as employees of Plantation Farm Camp.

2. Employee At-Will Acknowledgment: Download, complete, and mail to us as soon as possible.

3. Health Form: You and your doctor must complete this Health Form. If you have not had a physical examination within the two years before the last day of Camp, you will first need to get a physical examination. This requirement for updated medical information is to protect you in case of illness or injury by making available to a medical provider current, useful information. Even if you are a returning staff member, we need an updated Health Form. As you will note, you will need to print the form to sign as well as for your doctor to complete and mail to us. Please return your form to us as soon as possible.  If you have a health form from CCUSA or another international agency, please fill out our health form as well, but you do not need a doctor to fill out our form too.

4. Substance Abuse Policy: Please download and read this policy.  Print, complete and mail back the Acknowledgement Form and Consent Form (pages 9 an 10).  These forms are required prior to the start of your employment.  All seasonal staff will be tested during orientation.

5. CPR Certifications: Forward to us copies of your certifications for CPR (which must be current through the end of Camp.)  If you have not received CPR certification yet, please arrange to do so before the beginning of staff training.

6. Other Certifications: Forward to us copies of any other certifications (i.e., lifeguard, CHA, and wilderness first aid/responder).

7. Suggested Packing List. We have included a packing list suggesting certain items you may wish to bring to Camp.  For international staff travelling, we can lend you some of these items.  Please contact us at camp before you arrive to discuss borrowing things.

8. Travel Arrangements: Please let us know (by email preferably) how and when you will be arriving. If you are driving and need directions, click here to download a map and directions. If you are flying into Oakland or San Francisco, you can take the Airport Express bus to the Petaluma Fairgrounds and we can pick you up from there.

9. Employment Agreement Letter: Your Employment Agreement will be emailed to you separately.  Sign and return a copy of your Agreement Letter to indicate that you have received all the information identified above, including the electronic forms through our Web site, that you have read and understood everything, and that you agree to the terms of employment. If for some reason you have difficulty downloading an electronic form, please let us know so that we can correct the problem or provide you with a paper copy.

State and Federal Information

10. Form W-4: Complete and forward to us as soon as possible.

11. EDD Forms 35 ("Notice to Employees"), 1875A ("Notice to Employees"), 1857D ("Notice to Employees Unemployment Insurance Benefits"), 2320 ("For Your Benefit"), 2511 ("Family Leave"), and 2515 ("State Disability Insurance Provisions"): We are required to provide you with these forms from the California Employment Development Department for your review and information. You do NOT need to sign and return any of these forms.

12. "New Employees Guide to Workers' Compensation": We are also required to give you information on California's Workers' Compensation system, and we provide this form for your review and information. If you wish to indicate the name of your personal physician or chiropractor, you may do so on this form. Otherwise, you need not complete and return this form.

13. "Employee's Guide to the State Fund Medical Provider Network (04/08)": We are also required to give you information on our workers' compensation insurance carrier's Medical Provider Network, and we provide this form for your review and information.

14. Application for Social Security Number: If you do not have a Social Security Number (i.e., you're an international staff member), you will need one before the Camp can pay you. If you are working through an agency such as CCUSA, they will help you obtain the Social Security Number. If you are not working through an agency, you must complete this application and assemble the required documents detailed below in order to get a Social Security Number. When you arrive in the U.S., you will take (1) the completed application, (2) your passport, (3) the pink copy of DS-2019 (formerly IAP-66) which will be given to you by our visa agency or your own agency, and (4) the white I-94 card (issued and placed in your passport when you enter through U.S. Customs) to the local Social Security office to apply for your Social Security Number. Contact the Camp about the location and timing for submitting your application.

15. Form I-9: Review this form, which is a form that the U.S. Immigration and Naturalization Service (INS) requires both employers and employees to sign. (If you are an international staff member, you only need to show us a copy of the J-1 Visa stamped in your passport.) You may, at your option, provide us with photocopies of the appropriate documents (ensuring they and any photos are legible), OR bring the originals to Camp for our inspection.
